Referral / Enquiry Recieved
(via phone, email, internet, fax, post, in person) Triage to determine suitable services
If not suitable for Rehabilitation
suggest suitable alternative services, communicate with relevant parties and close case.
RTW Rehab Makes Initial Contact With Parties Concerned
To understand current needs and organise assessment date
If intervention is required in the future
re-assess new needs and continue collaborating with all parties.
Perform Assessment
In order to determine health, lifestyle and work goals for example:
- Workplace Assessment
- Ergonomic Assessment
- Vocational Assessment
- Activities of Daily Living Assessment
Consult All Necessary Parties
Collate, Clarify, Consider and Understand Medical, Workplace and Personal needs.
If RTW Rehab Is Needed
For Ongoing/Further Services, We:
- Address issues as they arise, liaise and negotiate with all parties concerned
- Create Graduated Activity Plans to guide all parties to achieve end goals/outcomes
- Monitor on a daily/fortnightly basis and provide rehabilitation reports to all parties periodically
